CHILD HEALTH AND
DISABILITY PREVENTION PROGRAM:
The Child Health and
Disability Prevention Program provides physical assessments for preschool,
Head Start, kindergarten and first grade students as required by state law
for school entry. Physical assessments are available to students and
infants ages birth to 18 years with Medi-Cal or Medi-Cal Managed Care upon
request by a school development program within Stanislaus County.
Health assessments are
conducted by nurse practitioners at the school site, Head Start, or Child
Development Center in an available room within the school. Our CHDP Program
functions as CHDP provider under the jurisdiction of the County community
provider, Stanislaus County Health Services Agency. Close monitoring,
examination reviews, and health education conferences are provided by a
Pediatrician Consultant.
