Employee Injury Reporting Process
Medical Treatment for Work Related Injury/Illness
The Stanislaus County Office of Education (SCOE) has selected the following physicians and medical facilities to administer medical treatment for work related injuries. Please be sure to mention you are a (SCOE) employee and give correct work ad-dress. You must utilize one of these facilities unless you have designated your personal physician with the HR/Risk Management prior to the date of your injury.

Communication between medical providers, employees, and Human Resources/Risk Management is critical to ensure that proper care is administered and to address the employee’s ability to work. The listed medical facilities provide the level of communication required for successful injury management. SCOE has contractual agreements with these providers for reduced medical rates and by employees voluntarily choosing medical care from one of the providers, self-insuring workers’ compensation costs will remain the most cost efficient method of providing benefits to the injured workers. SCOE Driver Guidelines & Accident Reporting Procedures
Important procedures when you are operating a vehicle to conduct SCOE business and a vehicle accident occurs.
Please consult with your supervisor in regards to your eligibility of using a SCOE vehicle or your personal vehicle to conduct SCOE Business. Supervisors are to contact Safety Services with any questions that they may have in regards to driver eligibility and to ensure their employees are driver eligible.
Drivers are to comply with all State and Local laws, including all speed limits, traffic and seatbelt laws, and parking regulations. Under no circumstances should the driver operate a vehicle while under the influence of drugs or alcohol, or when their physical or mental condition may be otherwise impaired.
Accident Guidelines: SCOE vehicles are equipped with additional safety items such as a first aid kit, seat belt cutter, glass breaker, emergency reflectors, flashlight, fire extinguisher and an emergency reference guide.
Reporting Vehicles Accidents-9 Steps
- Make sure all people involved are ok.
- If someone is injured Call 9-1-1. If everyone is safe, report the accident to the proper law enforcement agency that has jurisdiction in the area where the accident occurred (Police or C.H.P.).
- Notify your supervisor immediately.
- Document the circumstances: take a picture of the vehicles involved, vehicle damage and the license plate.
- If you or another SCOE employee is injured, call Company Nurse for medical treatment. The number is on the back of employee badges. For serious injuries call 9-1-1 and have the supervisor contact Company Nurse with the incident details.
- Driver must file an SR-1 form regardless of who's at fault within 10 days if property damaged exceeds more than $1,000.00.
- Supervisor: Contact Human Resources/Safety Services to, medically clear any involved employees.
- Contact Robert Hallsteinsson at 541-2314 to make arrangements to have SCOE vehicle towed, fixed or assessed.
- Supervisor: Complete the property/liability loss form which is located in the emergency reference guide in the vehicle or contact Risk Management for a form. Send the completed form to Araceli Sanchez/Irene Alvarez, Risk Management.
If the accident involves your personal vehicle while conducting SCOE Business: Follow steps 1 to 7. Report the accident to your li ability insurance carrier and comply with all state or local reporting procedures.

Please Note:
- In the event of an emergency please dial 9-1-1. Supervisor is to contact Company Nurse after ambulance has been contacted.
- Do Not Delay Care - If the employee has received an injury where they cannot call in on their own, or treatment is needed immediately, Supervisor is to call Company Nurse. Employee to follow up with Company Nurse after care.
- Reminder: Most Non-Emergency MPN locations close at 5:00 pm. Please do not hesitate to call Company Nurse and seek treatment immediately.
- Company Nurse should be used for ALL employee injury reporting. If an employee has pre-designated, they need to let Company Nurse know upon initial call, then the employee must call Safety Services so we can verify we have the pre-designation on file. The injury information is then forwarded to Pegasus Risk Management electronically to set up a claim file so that benefits may be administered.
- A confirmation number will be provided at the end of each call. Write it down so you can reference it if you need to call back for any reason so that your report can be accessed.
- Company Nurse is NOT the claims administrator and is a separate and distinct company from Pegasus.

IPM Statement
It is the goal of Stanislaus County Office of Education to implement IPM by focusing on long-term prevention or suppression of pests through accurate pest identification, by frequent monitoring for pest presence, by applying appropriate action levels, and by making the habitat less conducive to pests using sanitation and mechanical and physical controls. Pesticides that are effective will be used in a manner that minimizes risks to people, property, and the environment, and only after other options have been shown ineffective.
Pest Management Objectives:
- Focus on long-term pest prevention using minimal pesticides.
- Elimination of significant threats caused by pests to the health and safety of students, staff and the public.
- Prevention of loss or damage to structures or property by pests.
- Protection of environmental quality inside and outside buildings, in playgrounds and athletic areas, and throughout the Stanislaus County Office of Education facilities.

Pest Management Contracting
Pest management services are contracted to a licensed pest control business, as needed. Pest Control Business name(s): Paramount Pest Control and Clark Pest Control.
Prior to entering into a contract, Stanislaus County Office of Education has confirmed that the pest control business understands the training requirement and other requirements of the Healthy Schools Act.
Pest Identification, Monitoring and Inspection
Pest identification is done by: Tanner Mendoza
Monitoring and inspecting for pests and conditions that lead to pest problems are done regularly by the Custodians and Groundskeepers. The results are communicated to the IPM Coordinator.
Specific information about monitoring and inspecting for pests, such as locations, times, or techniques include:
- Monitoring for vertebrate pest debris by custodians in all work areas.
- Utilizing sticky monitoring boards for vertebrate and non-vertebrate pest activity. These are checked by custodians and results communicated to the IPM Coordinator.
- Monitoring of gopher and other vertebrate mounds in playgrounds, athletic fields, and other landscaped areas. These results are communicated to the IPM Coordinator.
- Monitoring buildings for wasp and hornet activity by the site custodians. Any activity is reported to the IPM Coordinator.

Chemical Pest Management Practices
If non-chemical methods are ineffective, Stanislaus County Office of Education will consider pesticides only after careful monitoring indicates that they are needed according to pre-established action levels and will use pesticides that pose the least possible hazard and are effective in a manner that minimizes risks to people, property and the environment.

Healthy Schools Act
Stanislaus County Office of Education complies with the notification, posting, recordkeeping, and all other requirements of the Healthy Schools Act. (Education Code Sections 17608-17613, 48980.3; Food & Agricultural Code Sections 13180-13188)
Training
Every year, Stanislaus County Office of Education employees who make pesticide applications receive the following training prior to pesticide use:
- Pesticide specific safety training (Title 3 California Code of Regulations 6724) and
- School IPM training course approved by the Department of Pesticide Regulation (Education Code Section 16714; Food & Agricultural Code Section 13186.5) and located at: http://apps.cdpr.ca.gov/schoolipm/training/main.cfm.
Submittal of Pesticide Use Reports
Reports of all pesticides applied by Stanislaus County Office of Education staff during the calendar year, except pesticides exempt1 from HSA recordkeeping, are submitted to the Department of Pesticide Regulation at least annually, by January 30 of the following year, using the form provided at www.cdpr.ca.gov/schoolipm. (Education Code Section 16711)

Review
This IPM plan will be reviewed (and revised, if needed) at least annually to ensure that the information provided is still true and correct.
